Alberta Health Services home rehabilitation teams accepting clients in the South zone

Alberta Health Services’ South Zone Home Rehabilitation Teams are accepting new clients in Medicine Hat and Lethbridge.

Launched in 2017, HRTs work with clients who have recently experienced a decline in independence, function or mobility.

Teams are multi-disciplinary, comprised of health and wellness specialists in a variety of fields, including occupational, physical and recreation therapy, speech-language pathology, nutritional science, medicine and social work.

Over the course of four to 12 weeks, teams and clients work collaboratively to determine the client’s personal goals and develop a daily therapy plan to help them achieve such; with the ultimate goal of improving independence and reducing potential hospital stays and the likelihood of a move to a congregate living facility.

Since 2017, HRTs have supported more than 3,100 South zone residents.

HRT support is available following physician or self-referral and AHS priority assessment based on need.
